On 7/2312011 at about 3:45 PM. Officer Hickey was notitied by Inmate Boyd that Inmate Barker had died of natural causes. Inmate Barker was a hospice inmate assigned to lnfinnary bed IN-3. Inmate Boyd and Inmate Cohron were the assigned inmate Hospice volunteers. Both Inmate Boyd and Cohron were present at the bedside when Inmate Barker stopped breathing. Oft-ieer Hickey responded to the area and relieved RN Stevens and RN Maruna who had secured the scene. C.o. Hickey notified Mash..T Control. Master Control staff notified Capt Bellman. the Ole. Capt Bellman assigned Officer Contreras to take over the area and establish a crimI! scc:nc. The Medical Examiner. J. Moffitt arrived at 4:44 PM and released the body at 5:20PM. The State Police Detective. Sgt. Withers arrived at 5:40PM and cleared the scene at 6:04PM. Due to the extended amount of time until rhe ti.mcral home could respond to the institution. the body was moved by gurney and sccurcd in tbe Master Control sallyport. The-area remained secured until Alternative Burial and Cremation of Oregon arriv~d at 7:58PM and transported the body out of the institution. 4. I was informed by cia Hickey that RN Christina ivlihalyi-Maruna and RN Jenni!"er Stevens were noti lied by thc Hospice volunteer, Inmate Jimmie Cohron SID# 4577168, that Inmate Barker had expired. Both nurses responded to room 328 in the asp Intinnary. Inmate Cohron and Inmate Stuart Boyd, SID# 6952044, were present in the room when Inmate Barker expired as well as the ward orderly Inmate Whiting, Kevin SID# 10313531. Inmate Barker's roommate, Inmate 1·leath, James SID# 4357424, was in his assigned bed, bednuillber 4, at the time of IllIuate Barker's passing. He was removed to a new bed on the ward t1oor. At 5:00 pm, the Marion County iVfedieal Examiner, Jane Mortitt, arrived in the room and examined the body of Inmate Barker. At 5:07 pm, !vIE lvloftill declared that she was releasing the body. At 5:08 pm, she questioned Inmate Boyd and Imnate Cohron. RN Stevens notified IVIE Moffitt that Mr. Ted Randall had made all the necessary next of kin notifications. At 5:45 pm, State Police Detective Gregg C. Withers arrived and examined the body of Inmate Barker. He also questioned 111l11ate Boyd and Inmate Cohron as well as RN Stevens and RN lvlihalyi-Maruna. Del. Withers then cleared the crime scene and stated that the fhneral home could remove the remains of h1Jnate Barker. At 6:30 pm, the body ofInmate Barker was removed from the to Master Control for pick up by Alternative Funeral Home. On July 23, 2011 at 1545 hours Inmate Boyd, Stuart #6952044 came to the Officer's station and informed me that Inmate Barker, Terry #3886090, assigned to Infirmary bed IN-03, had passed away. I walked towards the ward and RN Maruna confirmed that inmate Barker had passed away. Rn Stevens had already cleared the rooms of inmate Heath #4357424 assigned IN-04, Inmate Boyd #6952044 assigned inmate Hospice care, inmate Cohron #4577168, assigned Hospice while scheduling a vigil, and inmate Juras, Andrew, assigned Infirmary Orderly. While RN Stevens stayed at the doorway to the side room for IN-03 and IN-04 I returned to the Officer station and notified Master Control by telephone of the death. of inmate Barl<er. I then conducted count and relieved - RN Stevens Recording who was in the room and starting a crime scene log at 1600 until I was relieved by CO Contrases at 1610 to continue my duties as the Infirmary Officer... I immediately went to infirmary bed #3 and assessed Terry Barker. He was found to be sitting up in bed, no respirations, no apical or radial pulse, and pupils dilated. Hospice volunteer Jimmie Cohron #4577168 was at bed side #4 with James Heath #4357424 and hospice volunteer Stuart Boyd #6952044 was standing just outside the room. Inmate orderly Andrew Juras #12942661 was present in room. I asked 11M Juras and Cohron to assist 11M Heath out of the room to the day area. I placed a privacy screen in front of window with the assistance of Nurse Maruna. Once the room was cleared, I proceeded to stand at room entry until relieved by C/O Hickey. At 1545 Jimmie Cohron #4577168 told me that Barker did not appear to be breathing. I went into the room and found Mr Barker sitting up in bed with his head slumped forward. I was unable to hear an apical pulse or breath sounds with my stethescope .
