On the above date I was assigned to the Infirmary as special security detail for the visitor of Inmate
GAARSLAND, WALLACE #8256438. At approximately 0545 hI's, I noticed that Inmate Gaarsland had stopped

breathing. I immediately notified nursing staff. RN's Sabrina McCain and Mark Ebner went into the room to
check on Gaarsland. At 0550 hI's, both nurses pronounced Inmate Gaarsland dead. END.

INTEROFFICE MEMO

On 3-24-11 at 6:02am I was assigned to be the Crime Scene Officer after Inmate
Gaarsland, Wallace SID# 8256438 passed away. Rick Thompson, Chief Medical Examiner
released the body at 7:26am. I escorted the body from Infirmary room #1 down to R&D where it
was released to funeral home staff at 8:35am. At no time was the body tampered with.

SUBJECT: Infirmaryl Hospice Vigil

On March 24, 2011, at 6:00am I was assigned to escort as well as comfort an outside Infirmary
guest Tareena, who was on a Hospice Vigil for Inmate Wallace Gaarsland SID#8256438. Inmate
Gaarsland had just passed away. I sat with Tareena, until she felt she was fine, and was able to
drive herself safely home. I escorted her out of the Institution at 7:30am.
