Contract No.CDOC6840 Amendment No. 5 State of Washington Department of Corrections This Amendment to Contract No. CDOC6840 is made by the Washington State Department of Corrections and Value‐Added Communications, Inc. for the purposes set forth herein. WHEREAS Value‐Added Communications, Inc. (VAC) has implemented new rates charged offenders for prepaid or collect interstate phone calls; and WHEREAS the Department of Corrections (DOC) and VAC have reached consensus on a reduction in the rates for instate offender phone calls; NOW THEREFORE, in consideration of the terms and conditions contained herein the Department and VAC agree as follows: 1. VAC will reduce the flat rates charged for local, intraLata, and interLata calls made by offenders effective April 1, 2014. Collect calls made by offenders that are local, intraLata, and interLata will cost $3.04 for a twenty minute call. Pre‐paid calls made by offenders that are local, intraLata, and interLata calls will cost $2.80 for a twenty minute call. Call costs do not include any applicable taxes or charges approved by the Federal Communications Commission or the state Public Utilities Commission. 2. The minimum amount guaranteed DOC under Section 3.02 and Appendix 3.01 of this Contract is reduced to (MAG) is reduced to $4,028,400. All other terms and conditions remain in full force and effect. The effective date of this amendment is April 1, 2014. THIS AMENDMENT, consisting of one page is executed by the persons signing below who warrant that they have the authority to execute the contract amendment.
